Research in rehabilitation medicine presents unique ethical challenges related to patient capacity, communication, potential subject overuse, timing of recruitment, hope for a cure, therapeutic misconception, and the provider-subject relationship. Many rehabilitation patients have cognitive deficits impacting their decision-making capacity, requiring careful assessment to ensure truly informed and voluntary consent before participating in research. The strong bond between rehabilitation patients and their healthcare providers can lead patients to participate in research out of a desire to please their care team, necessitating extra caution to ensure voluntary participation.
Develop standardized capacity assessment protocols for rehabilitation patients with potential cognitive deficits.
Incorporate speech and language pathologists in the research process to aid patients with aphasia and other communication barriers.
Establish guidelines for the timing of subject recruitment, ensuring patients have sufficient time to adjust to their condition before participating in research.
